IUDs | Brigham Wellesley OB/GYN Associates

Choosing a birth control method is deeply personal, and it should always feel like your choice, never something that you’re talked into or prescribed without having all the information surrounding your choices. At Brigham Wellesley OB/GYN Associates, we know that navigating contraception options can feel overwhelming, especially with so much conflicting information out there. That’s why we take the time to talk through what actually matters for your body, your lifestyle, and your goals, so you can choose with confidence.

One option many of our patients ask about is the IUD, or intrauterine device, which is a small, highly effective form of long-term birth control that’s placed directly into the uterus. If you’re considering an IUD, or simply want to understand your options better, we’re here to walk you through everything honestly and without judgment.

What Is an IUD?

An IUD is a small, T-shaped device inserted into the uterus by your provider to prevent pregnancy. Once in place, it works continuously in the background, requiring no daily attention, no monthly prescription refills, and no interruption to intimacy. It’s one of the most effective forms of reversible birth control available, with effectiveness rates that rival permanent sterilization, all while remaining fully reversible whenever you’re ready to have it removed.

IUDs generally fall into two categories:

Hormonal IUDs

Hormonal IUDs release a small, steady amount of progestin directly into the uterus. This thickens cervical mucus to prevent sperm from reaching an egg and often thins the uterine lining, which can significantly lighten periods or stop them altogether for some patients. Hormonal IUDs are typically effective for three to eight years, depending on the specific device.

Copper (non-hormonal) IUDs

Non-hormonal IUDs contain no hormones at all. Instead, copper ions create an environment that’s inhospitable to sperm, preventing fertilization. Because they’re hormone-free, copper IUDs don’t affect your bleeding cycle, though some patients do experience heavier or longer periods, particularly in the first few months. Copper IUDs can remain effective for up to 10 years or longer.

Who Is a Good Candidate for an IUD?

IUDs are a good option for many patients, including those who:

  • Want a highly effective, low-maintenance form of birth control
  • Prefer not to think about contraception on a daily or monthly basis
  • Are looking for a reversible alternative to permanent sterilization
  • Want to avoid or minimize hormonal side effects (copper IUD)
  • Hope to reduce heavy or painful periods (hormonal IUD)
  • Are breastfeeding, since most IUDs are safe during lactation
  • Have completed their family but aren’t ready for a permanent decision

IUDs aren’t the right fit for everyone, and that’s completely okay. During your consultation, we’ll review your medical history, any current conditions, and your personal preferences to determine whether an IUD makes sense for you, or whether another method might work better for your goals.

What to Expect During IUD Insertion?

We know that insertion is often the part patients feel most anxious about, so we will discuss it honestly. The procedure itself is brief, typically taking just a few minutes, and is usually done right in our office.

Before insertion, your provider will perform a pelvic exam to check the size and position of your uterus. A speculum is then used, similar to a Pap test, to allow clear access to the cervix. Your provider will gently insert the IUD through the cervix and into the uterus using a thin, flexible applicator tube. Once positioned, the applicator is removed, leaving only the IUD in place, with two thin strings extending slightly into the vagina for future removal or string checks.

Many patients experience cramping during and shortly after insertion, similar to strong period cramps. This typically subsides within a few hours to a couple of days. We encourage patients to take an over-the-counter pain reliever beforehand if needed, and we’re always happy to discuss additional comfort measures during your appointment.

What to Expect from Life with an IUD?

Once your IUD is in place, there’s very little you need to do. We recommend periodically checking for the strings to confirm the device remains properly positioned, and you’ll have a follow-up visit 4-6 weeks after insertion to make sure everything looks as it should.

Some patients notice changes to their cycle in the first few months, particularly spotting or irregular bleeding with hormonal IUDs, or heavier periods with copper IUDs. For most patients, these mild side effects settle within three to six months. Beyond that adjustment period, many patients describe their IUD as essentially invisible in daily life. There are no pills to remember, no appointments to schedule, just reliable protection working quietly in the background.

Removing or Replacing Your IUD

An IUD can be removed at any time by your provider, whether because you’re ready to start a family, you’d like to switch methods, or your device has simply reached the end of its effective lifespan. Removal is typically quicker and more comfortable than insertion, and fertility generally returns quickly afterward for patients hoping to conceive.

Why Choose Brigham Wellesley OB/GYN Associates for Your IUD Insertion?

Choosing where to have your IUD placed matters just as much as choosing the device itself. Our physicians perform IUD insertions and removals regularly, bringing both clinical skill and a calm, reassuring approach to an appointment that can otherwise feel intimidating. We take the time to answer your questions before, during, and after your procedure, and we never rush a conversation about your body or your birth control choices.

Most patients experience cramping during insertion that feels similar to strong menstrual cramps. It’s typically brief, and discomfort usually eases within a day or two. We provide a variety of pain management options, including nitrous oxide, for patients concerned about tolerating the procedure.

Yes. Your provider can remove your IUD at any time, and fertility generally returns quickly, often within the same cycle.

Will an IUD protect me from STIs?

No. IUDs are highly effective at preventing pregnancy but do not protect against sexually transmitted infections. Condoms remain the best protection against STIs.

Depending on the type, IUDs can remain effective anywhere from three to ten years or more. Your provider can help you choose the option that fits your timeline.
